You need to have Adobe Acrobat Reader installed on your computer to open this file. If you do not have this software installed on your computer, download a free copy of Adobe Acrobat Reader now. ASCAP writers and publishers can use the following forms to notify ASCAP of their intention to resign from ASCAP membership if online submission is not possible:

• Writer Resignation Form *(PDF)
• Publisher Resignation Form *(PDF)

After completing and signing the appropriate resignation form, it must be sent via certified mail to the following address:
